Add commentary and licence notices.
Some code in the main program is reordered too, but with no semantic
change.
Provide credit to van Oorschot and Wiener, and Teske, for their
algorithms.

rho.cc: Fix the cycle-finding table initialization.
My initial cut was hopeless, but Teske's suggestion doesn't work either
for trivial logs. This approach does.

For some reason the old code didn't actually break, but it seems like a
bad idea to try.

rhodes: Clear out debris from stale workers in `check'.
Doing this in every `step' leads to contention over the database, which
currently isn't handled well.
